Service overview

Sonke Support provides community access for NDIS participants across selected Sunshine Coast communities and the Bribie Island/Ningi area. This support can help participants get to appointments, use local spaces, keep routines, and take part in ordinary community life with the right level of assistance.

Connecting Through Community Access

Community access may include planned outings, shopping, appointments, libraries, parks, recreational facilities, volunteering, exercise routines, or social activities. Support is shaped around the participant's goals, transport needs, sensory and communication preferences, and the places that matter in their week.

Everyday Participation Made Possible

Some participants need practical help with planning and transport. Others need support to manage transitions, communication, confidence, or safety while out in the community. We focus on regular support that can become familiar and predictable over time.

Community Access across selected local areas

Sonke considers referrals where the participant's location, support pattern, goals, and staff match make regular support realistic.
