This is the main pathway for support coordinators, families, nominees, guardians, participants, and other professionals to refer or enquire about regular NDIS support with Sonke Support.

We focus on community access, in-home and daily living support, household routines, and practical life-skills support, with adult general support considered where the fit is right.

Current priority pockets include Bribie Island, Bongaree, and Ningi. For Caboolture, Morayfield, and nearby enquiries, use the Caboolture local support page before submitting the referral. Selected North Brisbane referrals are considered where schedule, travel, and consistent worker fit are realistic.

Regular support built around fit, routine, and consistency

Sonke may be a good fit where a participant needs steady, relationship-based support with everyday routines, community access, or practical skill building. We are especially focused on children and teens with autism or complex support needs where consistency, communication, and fit matter.

We also consider adult general support referrals where the location, support goals, schedule, and available staff match well.

What this referral pathway is not for

Sonke is not currently advertising SIL, respite, group activities, therapy, plan management, support coordination, crisis response, or public RRP marketing through this pathway. If the referral involves restrictive practices, we only consider support where those practices are governed by a current behaviour support plan and the required authorisations are in place.

This pathway is not for SIL, respite, group activities, therapy, plan management, support coordination, crisis response, or public RRP marketing. General enquiries can still be sent, but Sonke will not present those as advertised services.

Does Sonke Support provide behaviour support plan implementation?

If a referral involves an existing behaviour support plan, include that context so Sonke can confirm whether the requested implementation support is within its current service scope, authorisations, staff competency, and capacity before proceeding. Sonke does not advertise behaviour support plan development through this pathway.

Is the referral form for urgent or crisis situations?

No. The website and referral form are not monitored as emergency or crisis response channels. In an emergency, call 000. For urgent health, safety, or safeguarding concerns, contact the relevant emergency, health, or crisis service.

Behaviour support plan implementation

If a referral involves an existing behaviour support plan, include that context so Sonke can confirm whether the requested implementation support is within its current service scope, authorisations, staff competency, and capacity before proceeding.

We are not currently advertising behaviour support plan development through this referral pathway.

Restrictive practices are only considered where they are governed by a current behaviour support plan and the required authorisations are in place.
